COS Community Transition & Aging-in-Place Partnerships™ provides environmental assessment, home restoration, functional improvement, and ongoing environmental support for organizations serving individuals who are transitioning from institutional care back into the community, aging in place, or experiencing environmental conditions that interfere with the everyday use of their home.

COS works with referring and funding organizations to identify environmental needs, establish priorities, develop an appropriate scope of work, and restore home environments to greater function, order, comfort, and usability.

The COS Approach™

Every home tells us something about how it is functioning.

Rather than looking only at clutter, COS evaluates the whole environment to identify conditions affecting everyday use of the home.

Depending on the project, COS may observe severe accumulation and environmental congestion, inaccessible or poorly functioning rooms, disrupted pathways and furniture placement, storage breakdown, surface accumulation, underused or unusable living areas, environmental neglect, household systems that are no longer functioning effectively, and conditions requiring significant environmental restoration.

We then use the COS Method™ to move the environment through a structured process:

Read the Environment™ → Identify the Signals™ → Establish Priorities → Restore Function → Restore Order → Improve the Environment → Support Maintenance


COS Transition Home Assessment™

Starting at $497

Every intensive transition or restoration project begins with understanding the environment.

The COS Transition Home Assessment™ evaluates the current condition and function of the home and helps determine the appropriate level of environmental support.

The assessment may include review of selected living areas, room function, pathways and circulation, furniture placement, storage and accumulation, environmental congestion, everyday usability, visible maintenance or restoration needs, and priority areas.

Following the assessment, COS develops recommendations and, when appropriate, a proposed scope of work for the referring or funding organization.

Assessment does not automatically include implementation or restoration services.


COS Transition Home Reset™

Targeted Environmental Improvement

For homes that do not require extensive whole-home restoration, COS can provide focused environmental improvement to selected priority areas.

Projects may include restoring room function, improving pathways and furniture placement, reducing environmental congestion, establishing functional zones, improving organization and storage, resetting essential living areas, and implementing approved environmental recommendations.

Pricing is customized following assessment and scope approval.


COS Whole-Home Environmental Restoration™

Intensive Multi-Room & Whole-Home Projects

Some environments require substantially more than a traditional organizing session.

COS Whole-Home Environmental Restoration™ is designed for homes experiencing severe accumulation, extensive environmental congestion, multiple unusable or poorly functioning rooms, long-standing environmental neglect, or other significant conditions requiring a coordinated restoration process.

Projects may require multiple days or weeks depending upon the size and condition of the environment.

COS develops a customized restoration plan identifying priority areas, project scope, anticipated duration, environmental improvement objectives, and project pricing before work begins.

Whole-home restoration projects are individually quoted following assessment.


COS Aging-in-Place Environmental Improvement™

Helping the Home Better Support Everyday Living

A home that once functioned well may no longer support the needs of the person living there.

COS evaluates and improves environmental conditions that may make everyday use of the home more difficult.

Services may address room function, furniture placement, pathways, frequently used items, storage systems, environmental congestion, functional zones, comfort, visual conditions, and other environmental improvements within COS's scope.

Recommendations requiring medical, accessibility, construction, electrical, plumbing, structural, or other specialized expertise are referred back to the appropriate qualified professional.

Pricing is customized based on the environment and approved scope of work.


COS Community Partnership Agreements™

An Environmental Resource for the People You Serve

Organizations needing environmental services for multiple participants may establish an ongoing relationship with COS.

Partnership arrangements may include participant referrals, environmental assessments, transition-home projects, whole-home environmental restoration, aging-in-place environmental improvements, documentation of environmental findings, project recommendations, and ongoing environmental support.

Partnership structure and pricing are customized according to program requirements, referral volume, geographic service area, documentation requirements, project scope, and funding arrangements.


Prior Experience That Built This Work

The work behind COS Community Transition & Aging-in-Place Partnerships™ is not theoretical.

Before the development of the current COS Method™, our founder provided intensive environmental restoration services through an Area Agency on Aging, working in severely compromised home environments for individuals preparing to transition from nursing-facility care back into their homes.

Some projects required extensive, whole-home restoration over multiple weeks.

That experience helped shape a larger understanding:

Clutter is not always the entire problem. It can be one visible signal within a larger environmental system.

Today, the COS Method™ provides a structured framework for reading those conditions, identifying environmental priorities, restoring function and improving how the home supports everyday living.


How a COS Referral Works™

1. Organization Contacts COS
The referring organization contacts COS regarding a participant or environmental need.

2. Environmental Assessment
COS evaluates the home environment within the approved scope.

3. Findings & Scope Development
Environmental priorities are identified and the appropriate level of service is recommended.

4. Organization Approval
Project scope, pricing, responsibilities and applicable funding arrangements are approved before implementation begins.

5. Environmental Restoration or Improvement
COS performs the approved environmental work.

6. Completion & Recommendations
The referring organization receives appropriate project documentation and recommendations for maintaining the improvements or addressing additional environmental needs.


Important Scope Information

COS provides environmental observation, organization, functional space improvement, environmental restoration, space planning, and related non-clinical environmental services.

COS services do not replace medical assessment, behavioral-health treatment, occupational or physical therapy, social-work evaluation, building inspection, accessibility certification, environmental-hazard testing, biohazard remediation, pest-control services, licensed construction, or regulatory/compliance inspections.

COS does not diagnose hoarding disorder or determine whether an individual is medically or clinically cleared to return home.

When conditions are identified that require services outside the COS scope, the referring organization may be advised to involve the appropriate qualified professional or service provider before COS work proceeds.
